It is recommended that you use the online registration, however if you are registering after the cut off date of 5 days before each clinic, download these forms and bring them with you to the foreclosure clinic. Arrive early because you may still have an opportunity to see a counselor. We will serve the first 300 households that arrive.

Authorization to Release Information This form gives us authorization to speak to your bank on your behalf.
Form required for the clinic. If you have already registered through the website this is not required. If you are unsure, download and print this form and bring it with you to the clinic.
This form contains a list of documents you should bring with you to your counseling appointment.
This form will ask you to fill out your monthly income and expenses and will be used as a basis to determine what kind of options you have.
